We need you

Working closely with both our Aboriginal Health Practitioners and Doctors, our Registered Nurses are critical to our client's success. You’ll be committed to finding the best way to communicate with clients in order to provide great care and follow up with them in sometimes uncertain circumstances. You will need to provide high quality, culturally appropriate and effective nursing practice and improve the health and well-being of the Aboriginal & Torres Strait Islander people at Galiwin'ku within Elcho Island, NT. 

Core aspects of your role will include:

  • Following the best practice guidelines in the CARPA manual, work collaboratively with the clients, the GP and Allied Health staff to develop a management plan with the goal of chronic disease self-management.
  • Work collaboratively with clients and their support persons (families, carers) to achieve the client’s goals in the management of their chronic condition(s).
  • Ensure that all chronic disease clients have a current GP management plan and Team Care arrangement.
  • Prioritise and follow up chronic disease clients as per the guidelines in the CARPA manual and their GP management plan.
  • Complete all administrative follow up requirements.
  • Work collaboratively with other team members, including external organizations to enhance and support improved planning and continuity of care across service boundaries for individual clients. This includes interconnecting principles of equity, access empowerment, community self-determination and inter sectoral collaboration.

A competitive salary of $110,606 - $118,826 per annum (depending on skills and experience) plus superannuation and a comprehensive range of benefits including subsidised accommodation are provided with this full-time position.

  • Registered to practice in the NT as a Registered Nurse.
  • The demonstrated ability to work productively and co-operatively in a multi-disciplinary team.
  • Excellent communication skills and the ability to manage people so as to achieve a friendly and co-operative work atmosphere.
  • Skills in written and Information Technology on the use of computerized Patient Information systems.
  • A broad understanding of the underlying factors impacting on the health of Aboriginal people in remote areas.
  • A demonstrated understanding of the evidence base for addressing preventable chronic diseases in remote areas, including both prevention and management.
  • A broad understanding of the underlying risk factors impacting on the health of Aboriginal people in remote areas.
  • Ability and willingness to participate in the development of community-based primary health care programs.
  • Able to obtain a Working With Children’s Card (OCHRE card) and police check.
  • Hold current or eligible to obtain CPR and AGV certificates.
  • Current NT drivers’ licence.
