We need you

Working closely with both our Aboriginal Health Practitioners and Doctors, our Registered Nurses are critical to our client's success. You’ll be committed to finding the best way to communicate with clients in order to provide great care and follow up with them in sometimes uncertain circumstances. You will need to provide high quality, culturally appropriate and effective nursing practice and improve the health and well-being of the Aboriginal & Torres Strait Islander people at our Nhulunbuy Clinic, NT

Core aspects of your role will include:

  • Provide holistic care, support and maintenance in delivery of the chronic disease register/recall within the Bulunu region which includes Yirrkala, Gunyangara, Birritjimi and Nhulunbuy
  • Support clinics by working towards completing client list as generated by coordinator and clinic managers in ensuring KPI’s are met
  • In collaboration and autonomously work within framework of the Miwatj Chronic Disease Program.
  • Recognize and assists identification of hazards in the workplace
  • Complete all administrative follow up requirements
  • Work within the systems designed to ensure consultations are carried out for Chronic Disease clients as needed
  • Prioritising and actively following up of clients as per the allocated days
  • Provide advocacy and support for Chronic Disease self-management
  • Work collaboratively with Medical Officers, CD Coordinator, Allied Health workers, Acute Sector, Aboriginal Health Workers, Aboriginal Community Workers, project staff and administrative staff and other organisations to enhance and support improved planning( including compliance) and continuity of care across service boundaries for individuals clients
  • Ensure all chronic disease clients ,have a current GP Management Plan and Team Care Arrangement
  • Actively participate in Health promotion, planning and delivery of education, preventative chronic disease activities and community development
  • Involved in case conferencing, care planning and “needs for clients” situation
  • Support for clients attending specialist clinic as needed
  • Includes interconnecting principles of equity, access empowerment, community self-determination and inter sectoral collaboration
  • Adhere fully to MHAC Cultural Integrity Policy and attend Cultural Orientation

A competitive salary of $110,606 - $118,826 per annum (depending on skills and experience) plus superannuation provided with this full-time position.

  • Registered to practice in the NT as a Registered Nurse.
  • The demonstrated ability to work productively and co-operatively in a multi-disciplinary team.
  • Excellent communication skills and the ability to manage people so as to achieve a friendly and co-operative work atmosphere.
  • Skills in written and Information Technology on the use of computerized Patient Information systems
  • A broad understanding of the underlying factors impacting on the health of Aboriginal people in remote areas
  • A demonstrated understanding of the evidence base for addressing preventable chronic diseases in remote areas, including both prevention and management.
  • A broad understanding of the underlying risk factors impacting on the health of Aboriginal people in remote areas.
  • Ability and willingness to participate in the development of community-based primary health care programs.
  • Demonstrated personal qualities of motivation and commitment and ability to work autonomously with minimal supervision.
  • Current Australian driver’s license.
  • Possession of CPR, AGV certificates and Ochre card or eligible to obtain them.
